Difference between 5th and 4th editions
Side by side comparison of table of contents helps to figure out the most significant changes.

Key differences between 5th and 4th editions

  • Time lapse between current and previous publications: 13 years (2013 vs 2000).
  • Includes the fully revised diagnostic classification, as well as all of the diagnostic criteria from DSM-5 TM in an easy-to-use format.
  • The most remarkable structural change of the Fifth Edition is getting rid of 5-axial system
  • The new classification combines the axes together and rates the disorders by severity. The old classification grouped diagnoses down into independent dimensions called axes.
  • The diagnosis Mental Retardation is changed to intellectual developmental disorder.
  • Autism Spectrum Disorder is the new category for autistic disorder, Asperger disorder, childhood disintegrative disorder, and pervasive developmental disorder not otherwise specified.
  • Binge Eating Disorder is a new to the group - anorexia nervosa, bulimia nervosa, and eating disorder NOS - three different conditions which are placed in the Eating Disorders group.
  • Disruptive Mood Dysregulation Disorder (DMDD) is a new category.
  • New to the DSM-5 is bringing together obsessive-compulsive and other disorders previously arranged under category of Impulse Control Disorder into a broader class of disorders. Two new conditions added are excoriation (skin-picking) and hoarding disorder. In my opinion, that was a premature move.
  • In Personality Disorders category, all 10 disorders from DSM-IV remained, but the axes boundaries separating them from other psychiatric disorders were removed.
  • New chapter Trauma- and stressor-related disorders includes posttraumatic stress disorder (PTSD) and a new diagnostic sub-type for pre-school children.
